Jasmine is the Founder and Director of MaplePath Healthcare, bringing over 9 years of experience in the home healthcare industry. Throughout her career with Bayshore Home Health, she gained extensive experience supporting seniors and individuals with diverse care needs, while coordinating care, building meaningful relationships with clients and families, and ensuring high standards of service.
Her experience includes working closely with Personal Support Workers, nurses, healthcare professionals, hospitals, and community partners to help clients transition safely from hospital to home and maintain their independence. Jasmine has developed a deep understanding of dementia care, post-hospital recovery, chronic illness management, companionship, and personalized in-home support.
Driven by a genuine passion for helping others, Jasmine founded MaplePath Healthcare to provide a more personal and compassionate approach to home care. She believes every client deserves dignity, respect, and individualized care that enhances their quality of life while giving families confidence and peace of mind.
